CDSPI — Upholding Your Privacy Rights
CDSPI, as administrator
of the insurance and investment plans offered through the Canadian Dentists’
Insurance Program and the Canadian Dentists’ Investment Program
and a program of Affinity Services, and CDSPI Advisory Services Inc.,
an affiliate of CDSPI and a licensed insurance agency, must collect
and retain certain personal information in order to provide services
to you. Protecting your privacy is important to us. Accordingly, we
have established a Privacy Code, as well as Confidentiality Standards
for our employees, to ensure that your personal information remains
confidential.
Why We Require Personal Information
We require personal information in order to provide you with insurance
and investment products and services as well as affinity services, and
to comply with legal requirements. Our files contain a copy of your
application(s) for insurance coverage through the Canadian Dentists’
Insurance Program and investment products through the Canadian Dentists’
Investment Program along with any other information we receive which
is relevant to your application(s). Your insurance file may also include
correspondence with respect to your application or your coverage, medical
information about you and copies of correspondence or information filed
with respect to any insurance claims you make. Your investment file
may include correspondence relevant to your investment accounts. Your
Affinity Services files may include copies of your application for the
relevant affinity service, along with correspondence with the company
providing the affinity service.
How We Use This Information
We only collect information that is necessary to provide you with insurance,
investment and affinity services, and, subject to statutory requirements,
we retain this information only as long as is necessary to provide you
with these services. The information you provide us may also be used
for the collection of statistical information with respect to the Programs
and in order to inform you of other available products and services.
We do not sell our mailing list to other organizations. If you do not
want to be on our mailing list, please advise us in writing.
Who Has Access To This Information
Any information that you provide to CDSPI or CDSPI Advisory Services Inc. with respect to your insurance application or coverage will be
shared with the insurer offering that Plan through the Canadian Dentists’
Insurance Program. We share information relevant to your investment
application or investment account with the insurer that issues the segregated
funds for the Canadian Dentists’ Investment Program and its affiliated
companies that provide products and/or services for the Canadian Dentists’
Investment Program. Any information relevant to your Affinity Services
application is shared with the provider of the affinity service.
We restrict access to your personal information to those
of our employees who need such information to do their jobs. Those individuals
are subject to our Privacy Code and Confidentiality Standards which
specify rules for protecting the confidentiality of your personal information.
Use or disclosure of your personal information other than as described
above will only be made with your consent or where we are obliged or
permitted to do so by law or to protect our interests.
Your Access To Our Information
Subject to the exceptions* described below, you may review the personal
information contained in your file and, if necessary, ask for corrections
to be made to it, by making a written request to:

* We will not provide you with information that
may be subject to a legal claim of privilege. If your file contains
medical information about you that was not obtained directly from you,
that information may only be released to you by the insurer through
your physician. In addition, if you own insurance on the life of someone
else, such as your spouse, your file may contain information about the
other person. In such a case, we will only provide you with access to
the information about you unless the other person gives written consent
to our disclosure of their information to you.
