Participants in the Canadian Dentists’ Insurance Program can obtain helpful services from two sources — CDSPI and its affiliate company, CDSPI Advisory Services Inc.*

CDSPI’s Insurance Services Department can assist you with:
• General information about the Canadian Dentists’ Insurance Program’s plans
• Obtaining applications and other insurance forms by mail or fax
• Billing issues
• Information about the status of your existing Insurance Program coverage
• Change of address procedures
• Other administrative issues (such as a change of beneficiary)
